We’ve done our best to detail everything that is important n the processing of a decorated apparel order.  The info is as complete as we can make it in written form.  If you’d like more information about how we operate, please fell free to give us a call and we’d be happy to discuss any concerns you may have.


Sales Representative Profile Form

Filling out this form will help us understand who you are, your company structure and how it is you would like for us to communicate with you.  Some reps provide us with their cell phone number so we can send text messages, photos of just printed items, and other communication that is urgent in nature.  In certain circumstances that method of communication is very helpful.  Some of our distributors consist of one hard working individual who wears many hats while others have multiple sales reps who all send us orders.  Some distributors are affiliates of larger organizations and others are independent.  Some have inside support staff, art staff and whole departments to handle order details, approvals, invoicing, etc.  Fill out this form and give us a clearer picture of who you are.


Payment Terms and Credit Application Form

Every distributor who sends an order to Boardwalk will be asked to fill out this form so we can understand how you would like to pay for our services.  If you prefer to pay by credit card, you can give us your card information here.  If you prefer to apply for open account this form contains our credit application.  And if you are interested in saving a little money by paying via ACH, there are more details inside.
